Job Summary
The RN-Case Manager conducts in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illness, visiting weekly for 1-2 hours to assess vitals, ensure home safety with medical equipment, and manage weekly medication planners while overseeing skilled and non-skilled caregivers. Responsibilities include providing direct care and case management, planning and evaluating care, coordinating with the regional director and staff, participating in client-care conferences, supervising clinical personnel, ensuring documentation accuracy, and contributing to performance improvement initiatives. The role supports marketing activities as requested and requires collaboration with staffing coordinators to schedule personnel within scope of practice. Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- BSN preferred
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
