Registered Nurse (RN) Case Manager

Job Summary
Conduct nursing oversight and case management for individuals enrolled in the Texas Home and Community-based Services (HCS) Program. Complete initial and ongoing nursing assessments, develop individualized care plans, and coordinate medical appointments, hospital discharges, and follow-up care. Monitor medication administration, review physician orders, and ensure regulatory compliance through accurate electronic and paper medical records. Participate in Interdisciplinary Team meetings, assist with incident reviews, and provide training to Direct Support Professionals. Work within the agency's on-call rotation as needed.
Required Qualifications
- Current Texas Registered Nurse (RN) license in good standing
- Valid Texas driver's license and reliable transportation
- Current CPR certification
- Excellent communication and organizational skills
- Strong computer skills including Microsoft Office
- Ability to work independently while collaborating with a multidisciplinary team
- Active Registered Nurse (RM) License in Texas, in good standing
- Minimum of 1-2 years of nursing experience
- Valid driver's license and reliable transportation
- Ability to travel within assigned caseload area
Desired Qualifications
- Experience working with individuals with Intellectual and Developmental Disabilities (IDD)
- Experience in the HCS or related waiver programs
- Knowledge of Texas HHS and HHSC regulations
- Experience with RN Delegation and BON requirements
- Case management experience
- IDD, long term care, or community health
