Job Summary
Registered Nurse (RN) Case Manager provides skilled nursing care in patients' homes as ordered by the physician, conducts OASIS assessments to develop and adjust plans of care, administers medications as prescribed, helps decrease re-hospitalizations through proactive visits and care coordination, and collaborates with Aides and LPNs while ensuring accurate documentation and adherence to Medicare/federal guidelines. The role requires travel to patients' homes within the defined geographic area, driving with a valid license, and maintaining an active RN license in the state of employment. The position emphasizes patient and family education, discharge planning, multidisciplinary collaboration, and compliance with agency and regulatory requirements. The schedule is typically M-F, 8a-5p, with on-call requirements and opportunities for professional development and competitive benefits.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum of one year of experience as an RN in an acute care setting
