Registered Nurse (RN) Case Manager

As a Registered Nurse Case Manager (RNCM) providing home-based nursing care, you will assume leadership for the assessment of patient condition and plan of care, case manage under physician direction, educate patients and families, and supervise nurse aides to ensure safe, high-quality care. You will perform OASIS assessments to develop individualized care plans, coordinate multi-disciplinary care, minimize patient risk, promote continuity of care through appropriate admissions/transfers/discharges, and ensure documentation complies with agency and Medicare guidelines. The role emphasizes autonomous time management, collaboration with an interdisciplinary team, and delivering compassionate, patient- and family-centered care in the home setting.
