Job Summary
RN Case Manager (RNCM) in Summa Health at Home provides home-based nursing care, leads assessment of patient condition, develops individualized care plans, and coordinates multidisciplinary care under physician direction. Responsibilities include case management, educating patients and families, administering medications as prescribed, supervising total care provided by nurse aides and LPNs, conducting OASIS assessments, ensuring Medicare/Federal guidelines compliance, and maintaining continuity of care across admissions, transfers, and discharges. The role emphasizes autonomous work within the home-health setting, with interaction from an interdisciplinary team to improve patient outcomes and support patients in their preferred environment.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
- Valid driver s license and auto insurance in your name as a driver
- Capable of all physical demands
