Registered Nurse (RN) Case Manager

Assume ultimate accountability and leadership for patient condition assessment and care plan development. Provide case management and clinical care while coordinating all disciplines to deliver quality, cost-effective, and billable care through direct supervision. Educate patients and family members, ensuring patient safety and accurate documentation compliance with all legal regulations. Manage practices and duties within the scope of practice as outlined by the state. Maintain a disciplined work ethic with the ability to prioritize, handle situations with appropriate nursing judgments, and work effectively as a team member. Monday through Friday, 8:15am to 5:00pm, with evening, weekend, and holiday on-call rotation. Valid driver's license and auto insurance required.
