Job Summary
RN Case Manager position overseeing home-based clinical care: assess patient condition, develop and adjust plans of care, perform OASIS assessments, administer medications as prescribed, supervise nurse aides and LPNs, coordinate multi-disciplinary care, and educate patients and families while ensuring safe transitions home. Responsibilities include leading care under physician direction, maintaining Medicare/federal documentation, managing time efficiently, attending in-service trainings, and fulfilling on-call expectations within a five-site Ohio territory (Circleville, Hillsboro, Logan, Washington Court House, Chillicothe). Requires an active RN license, minimum one year of acute care experience, valid driver's license, and ability to meet physical demands; travel to patient homes is expected.
