Conduct comprehensive in-home assessments and skilled documentation for high-acuity patients, focusing on wound care and maintaining OASIS accuracy. Build and manage plans of care with physicians while supervising LPNs and HHAs. Drive low avoidable-hospitalization rates and strong patient-experience scores by participating in case conferences and team huddles. Serve as the primary referral-source ambassador to ensure physicians and discharge planners recommend your pod. Ramp to approximately 30 productivity points weekly with a full-time Monday–Friday schedule plus some weekends. Earn $64.10 per point plus mileage reimbursement and a $1k referral bonus.
