Job Summary
RN Case Manager responsible for in-home nursing visits for former nuclear weapons workers with chronic/terminal illness. Conduct weekly visits (1–2 hours), perform physical assessments, ensure home safety with medical equipment and supplies, manage medication planners, and oversee caregivers. Responsibilities include direct patient care, care coordination with regional director and staff, care planning and evaluation, coordinating client care conferences, delegating within policy, ensuring documentation accuracy, contributing to performance improvement initiatives, and supporting staffing and scheduling as needed. Opportunities to engage in marketing activities and participate in agency-wide quality initiatives; schedule is self-directed to support work-life balance. Requires active RN licensure and relevant nursing experience, with BSN preferred and CPR certification.
Required Qualifications
- Graduate of a state approved school of professional nursing
- Current, unrestricted RN license in state(s) of practice
- One year in home care or closely related field preferred
- One year of supervisory and/or case management experience preferred
- Current CPR certification
- BSN preferred
