Job Summary
Conduct 1:1 holistic support for patients and families in their homes while partnering with an interdisciplinary team to develop individualized care plans. Complete initial and ongoing comprehensive assessments to determine hospice needs, administer prescribed medications and treatments, and educate caregivers on care techniques and symptom management. Rotate on-call duties to ensure seamless care transitions and guide end-of-life conversations with professionalism and empathy. Requires 1+ years of recent acute care experience, valid driver's license, and reliable transportation. Full-time 8:00am to 5:00pm schedule with on-call expectations.
Required Qualifications
- Registered Nurse (RN)
- Associates, Bachelors, or Masters of an accredited nursing program
- 1+ years of recent acute care experience
- Must have reliable transportation
- Valid driver's license
- Proof of auto insurance
- Ability to pass DHS background study
