Registered Nurse (RN) - Case Management - St. Vincent Medical Center

Job Summary
Manage patient transitions of care from the hospital to appropriate post-acute settings while assuring third-party payer approval for all services provided. Ensure appropriate documentation to support patient status, level of care, and DRG assignments. Analyze clinical, financial, and social situations to identify problems and alternative courses of action, guiding the healthcare team to assure optimal outcomes. Requires a current Ohio RN license with at least three years of clinical experience and familiarity with DRG assignment. Includes weekend, holiday, and on-call rotation with 80 hours per pay period.
Required Qualifications
- Current RN license in the state of Ohio
- Must have a minimum of 3 years of clinical experience
- Case Management certification required within 3 years of hire date
- Current CPR certification
- Willingness to be flexible, resourceful, and creative in problem solving
- Ability to act independently and offer suggestions and new ideas for improving performance and operations
- Ability to guide and direct the health care team to assure optimal outcomes
- Clinical expertise in a specialty area
- Ability to organize and present information clearly and concisely
- Demonstrated interpersonal skills in order to effectively communicate with physicians, patients, families and other members of the interdisciplinary team
- Creative problem solving and critical thinking
- Excellent customer service skills
- Working knowledge of DRG assignment
- Weekend, holiday and on-call rotation required per department policy
Desired Qualifications
- BSN preferred, or must be obtained within 3 years
- Clinical specialty or case management certification preferred
