Registered Nurse (RN) - Case Management

Supervise and support direct care staff to ensure compliance with policies and regulations while managing a caseload of clients and developing personalized treatment plans. Conduct comprehensive assessments and supervisory visits, documenting care plans in line with agency standards. Lead orientation, training, and competency verification for new hires, and act as a liaison among staff, community, and healthcare providers to ensure seamless communication. Provide in-field supervision and real-time guidance to clinicians, participate in quality assurance and performance improvement initiatives, and support on-call operations for timely follow-up and accurate documentation. This full-time, daytime role includes regular local travel and occasional on-call duties for Angels of Care Pediatric Home Health.
