Monitor, examine, and evaluate patient conditions while delivering care plans that promote short- and long-term outcomes. Collaborate with the care team to coordinate daily patient flow, prioritize actions based on needs, and maintain accurate documentation of assessments and incidents. Serve as an educational resource for patients and families, listen to concerns, and communicate pertinent information to providers and staff in adherence to confidentiality standards. Participate in performance improvement initiatives, maintain professional licenses, and support shared governance through service on councils.
