Job Summary
Full-Time Registered Nurse (RN) in Shreveport/Bossier City, LA for hospice care. Responsibilities include performing initial and ongoing patient assessments, assisting in developing and implementing the Hospice Plan of Care, educating patients and families on the Plan of Care (including medications, dressing changes, oxygen, and hospice philosophy), acting as primary liaison between physician, patient/family and hospice team, maintaining updated medical records, conducting routine supervisory visits, and participating in agency call schedule. Must have RN license in Louisiana, CPR, EMR knowledge, and at least 2 years of RN experience in relevant settings; prior Home Health or Hospice experience preferred.
Required Qualifications
- Registered Nurse (RN) license required in Louisiana
- Minimum of two (2) years full-time RN experience in oncology, acute care, long-term care, hospice or home health care
- Two (2) years hospice care experience may be substituted for RN experience if as an LPN
- Current CPR certification
- Current automobile insurance and valid driver's license
- Knowledge of EMR
- Previous Home Health or Hospice experience preferred
