Job Summary
Perform initial and ongoing patient assessments and assist in developing the Hospice Plan of Care. Educate patients and families on medication administration, dressing changes, oxygen use, and hospice philosophy while acting as the primary liaison between physicians, patients, and the team. Maintain accurate medical records and conduct routine supervisory visits. Participate in the agency on-call schedule and adhere to compliance guidelines. Requires a Louisiana RN license, two years of direct patient care experience (hospice preferred), current CPR certification, EMR knowledge, and a valid driver's license. Full-time role in Shreveport, LA.
Required Qualifications
- Registered Nurse (RN) license required in State of Louisiana
- Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care
- Current CPR certification required
- Current automobile insurance and valid driver's license
- Knowledge of EMR is required
Desired Qualifications
- Previous Hospice experience preferred
- Compassionate RN with great time management skills
