Job Summary
The Care Navigation Team Leader will supervise the Care Navigation team (RNs, LPNs, Care Navigation Social Workers, and Support Agents), participate in the development and growth of the Care Navigation Program, and oversee onboarding and training of new staff. The role supports evening triage, contributes to quality and process improvement initiatives, and cross-trains to cover additional leadership roles within the Care Solutions Center (including On-Call, Admissions/Liaison, and Triage). Key duties emphasize leadership and coordination across a multidisciplinary team to ensure high-quality patient and referral-source experiences. Strong RN licensure in Ohio and hospice experience underpin the responsibilities, along with excellent communication, leadership, and technology skills to manage multiple priorities in a healthcare setting.
Required Qualifications
- Registered Nurse (RN) in the state of Ohio strongly preferred
- Minimum of two years’ hospice experience required
- Strong communication, leadership, and managerial skills
- Ability to lead a large, multidisciplinary team, including managing performance and disciplinary action when necessary
- Strong technology proficiency
- Proven ability to manage multiple priorities and operational demands
