Job Summary
Identify and prioritize patients requiring care management services using a holistic approach that addresses biopsychosocial, functional, cultural, spiritual, and financial factors. Collaborate with patients, caregivers, and the healthcare team to plan, implement, and evaluate options for seamless transitions across the continuum of care. Monitor and revise care plans as patient conditions change, while ensuring clear documentation of post-hospital needs, resource utilization, and evidence of family involvement. Facilitate discharge planning, advance care planning, and readmission prevention by removing barriers to timely testing and treatment, and support assertive, proactive care coordination to optimize cost-effective outcomes.
Required Qualifications
- Bachelor of Science in Nursing (preferred for BSMH, required for RSFH)
- BLS Basic Life Support – American Heart Association
- RN License in the state in which they are working or covered by compact
- Accredited Case Manager Certification (ACM) from American Case Management Association or Certified Case Manager (CCM) from Commission for Case Manager Certification or American Nurses Credentialing Center (ANCC)
- 1 year of experience in clinical setting
- Care management

- Working knowledge of local and state resources

Desired Qualifications
- 3 years of experience in an acute care clinical setting
- Ambulatory or post-acute, care coordination experience
- Nursing Case Management board certification
