Job Summary
Collaborate with clinic team members to provide evidence-based, patient-centered care for high-risk medically complex patients, those on disease registries, and individuals with chronic physical or intellectual disabilities. Act as a liaison with family members and external agencies to facilitate communication and support care transitions, including pediatric-to-adult shifts, recent inpatient discharges, and ED follow-ups. Stay current on clinical standards while assisting with program processes and educating patients and families on disease nature and proper treatment instructions. Maintain quality, safety, and infection control standards while utilizing analytical abilities to interpret data and implement solutions. Requires an Associate Degree in Nursing, current Ohio RN license, and CPR/BLS certification.
Required Qualifications
- Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program
- Current Ohio Registered Nurse license
- CPR/BLS certified
- Knowledge of triage procedures along with clinic, physician office and/or urgent care experience
- Ability to maintain quality, safety, and/or infection control standards
- Knowledge of appropriate procedures and standards for the administration of medications and patient care aids
- Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
- Ability to convey information both in writing and verbally
- High level of interpersonal skills to handle sensitive and confidential situations

- Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions
