Registered Nurse (RN) BMT Transplant Coordinator FT - Days- Augusta, GA

Coordinate care for transplant patients, living donors, and families by facilitating medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluations throughout the transplantation continuum. Provide individualized education, support, and case management services from pre-operative phases through post-discharge and lifelong care. Participate in multidisciplinary team meetings, patient care planning, and selection conferences while advocating for appropriate transplantation or donation options. Execute clinical aspects of the program including performance improvement, quality monitoring, and process improvement activities. Maintain accurate documentation in OTTR and EMR systems, ensuring HIPAA compliance and timely updates on patient health status. Adhere to Wellstar Health System policies, standards of work, and regulatory guidelines for transplantation and living donation.
