Job Summary
As the Registered Nurse (RN) in the ACCS program, provide direct and indirect nursing services, coordinate care planning, oversee medical and psychiatric monitoring, and collaborate with facilities and families. Responsibilities include nursing assessments, treatment planning, health history/status documentation, health/nutritional remediation recommendations, accompanying individuals to appointments, and leading health care management meetings. Requires MA RN license and valid driver's license; mileage reimbursement and community-based work included.
Required Qualifications
- Active Registered Nurse license in the state of Massachusetts
- Must have valid driver’s license and a reliable car
- Minimum 6 months in Human Service field preferred
