Job Summary
Registered Nurse on the ACT Team delivers community-based behavioral health support to adults with serious mental health and substance use challenges. Responsibilities include psychotropic medication management, comprehensive nursing assessments, care coordination with psychiatrists and therapists, crisis response, and documentation of care observations in the Welligent EMR. Work occurs in homes, shelters, and community settings with a multidisciplinary team, regular in-person meetings, and rotating on-call duties; travel within the community is required. The role emphasizes compassionate, person-centered care, advocacy for client choice, health management education, and connecting individuals to needed community resources. Benefits and a salary range of $75,000–$90,000 annually are provided.
Required Qualifications
- Active, unrestricted RN license in North Carolina
- Experience in behavioral health or mental health (preferred)
- Comfort working in community-based settings
- Valid driver’s license, reliable transportation, and vehicle insurance
- Strong communication, documentation, and relationship-building abilities
