Job Summary
Assess individuals upon admission and complete ongoing nursing assessments while administering medications, vaccinations, and physician-ordered treatments. Monitor health conditions, coordinate medical and psychiatric appointments, and triage concerns by communicating with physicians. Educate Direct Support Professionals on medication administration and infection prevention, conduct medication room and EHR audits, and assist with care planning. Travel to assigned homes to provide skilled nursing care and ensure compliance with HIPAA and state regulations. Serve as a clinical resource to interdisciplinary teams while supporting infection prevention initiatives.
Required Qualifications
- Active Registered Nurse (RN) license
- Valid driver's license with the ability to travel locally between residential locations
- Proficient with electronic medical records and computer systems
- Must successfully pass required background screenings
Desired Qualifications
- Minimum of two years of nursing experience preferred
- Experience working with individuals with developmental disabilities or behavioral health needs is a plus
