Registered Nurse (RN) - Barry

Conduct patient assessments and provide education and instruction through symptom-based telephone and in-person triage. Schedule appointments, advise patients and parents on care plans, and ensure appropriate follow-up call-backs are made. Collaborate with interdisciplinary teams to coordinate services with other health-related agencies and resources. Document nursing actions in medical records, contribute to policy development, and participate in the quality assurance program. Provide translation services when needed and adhere to organizational compliance standards. Work Monday through Friday 8am to 5pm onsite at the Barry Community Health Center, with occasional Saturday mornings required.
