Job Summary
Registered Nurse (RN) responsible for administering the Care Plan and patient health per the plan, ensuring compliance with nursing/clinical standards and applicable regulations. Treat patients for Skilled Nursing Services as needed; collaborate with Director of Nursing, physicians, patient/caregivers, and care team; conduct on-site Orientation and quarterly supervision visits for home health aides; provide ongoing instruction to aides on Standard Precautions, Client Respect, HIPAA and Safety; review Personal Care Plans from Aging Service Access Points and orient aides accordingly; document services rendered and changes in client status; communicate with ASAP Case Managers or family about client status; perform other duties as assigned. Requires graduation from an approved nursing program and active RN licensure; 1+ years of home health experience preferred; valid driver's license and transportation; CPR preferred.
Required Qualifications
- Graduate of an approved school of professional nursing.
- Current licensed Registered Nurse in practicing state.
- One or more years of experience in clinical home health services, or equivalent preferred.
