Job Summary
Plan, coordinate, and ensure quality care services for patients and families in the home setting. Deliver professional nursing services, provide case management for primary patients, and monitor changing client needs and clinical outcomes. Develop work methods and procedures while maintaining ongoing communication with patients, families, physicians, and interdisciplinary teams. Implement and continuously monitor the plan of care, complete clinical documentation including OASIS evaluations, and function as a liaison with community health resources. Requires an active Registered Nurse license, minimum one-year clinical experience, and a valid driver's license. Flexible scheduling and local patient assignments available.
