Job Summary
Provide one-on-one skilled nursing care in patients' homes as directed by an attending physician, utilizing leadership skills to coordinate care and work at the top of your license. Observe and monitor patient conditions to perform OASIS assessments, develop individualized care plans, and adjust them as needs change. Administer medications as prescribed, prioritize visits for high-risk patients to minimize re-hospitalizations, and educate families on disease processes using teach-back methods. Complete all clinical documentation following agency protocol and Medicare guidelines while collaborating with interdisciplinary teams for optimal care. Work autonomously with efficient time management, understanding and following agency policies, procedures, and rules.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
