Job Summary
Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, and care plan evaluation. Obtain data from physical, psychological, social, and spiritual factors to initiate interdisciplinary interventions. Coordinate optimal care by communicating with attending physicians, agencies, and team members. Provide regular visits to re-assess needs and update plans. Perform clinical pronouncement of death and support families. Maintain accurate records, charts, progress notes, and reports. Supervise and document Home Health Aide delivery of care per Federal, State, and Company guidelines. Full-time Monday-Friday 8a-5p with rotating on-call.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR, First Aid Certification
- Knowledge of quality assessment and assurance procedures
- Valid driver's license and proof of insurance
