Job Summary
Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation. Obtain data from physical, psychological, social, and spiritual factors to initiate interdisciplinary interventions. Coordinate care with attending physicians and agencies, provide regular visits to re-assess needs and update care plans, and perform clinical pronouncements of death with family support. Maintain accurate records, charts, progress notes, and reports while supervising Home Health Aide delivery of care per guidelines. Work Monday-Friday 8a-5p with rotating on-call; eligible for Hospice Certification salary increase and tuition reimbursement.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR, First Aid Certification
- Knowledge of quality assessment and assurance procedures
- Valid driver's license and proof of insurance
