Registered Nurse (RN) - Acute Rehab Unit

Assess, plan, implement, and evaluate care for a given population based on age-specific and cultural needs using a collaborative multidisciplinary approach. Measure health outcomes against patient care goals, coordinate delivery of care for the Rehab Unit, and facilitate problem-solving with staff. Perform plan of care interventions including medication administration, specimen collection, and clinical treatments while documenting all patient care. Monitor and communicate patient conditions to the care team, physician, and family, and assist patients with activities of daily living such as hygiene, nutrition, and ambulation. Evaluate learning needs and provide patient education appropriate to age and culture, acting as an advocate for physical and emotional well-being.
