Registered Nurse (RN)-Acute Care, Long Term Acute Care, LTACH, PRN, Varied Shifts

Lead clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient/family-centered care using evidence-based practices. Conduct comprehensive nursing assessments, develop individual plans of care in collaboration with the interdisciplinary team, and validate goals with patients and families. Provide direct care to assigned patients, monitor quality, and perform quality improvement audits while ensuring patient safety and adherence to safety protocols. Coordinate discharge planning to assure continuity of care and smooth transitions between levels of care. Communicate patient condition changes via shift-to-shift reports and discuss outcomes with peers. Maintain professional licensure, complete mandatory education, and adhere to all compliance policies including infection control and medication administration. Schedule may include weekends, holidays, and on-call work.
