Registered Nurse (RN)-Acute Care- Care Coordination Sr. (ED 12 Hour Daytime)

Assess transitional care needs, coordinate care across the continuum, and engage with patients and families to assure care needs are met for complex cases during acute admission. Collaborate with physicians, utilization review nurses, and interdisciplinary teams to drive optimal treatment plans, streamline care progression, and plan for post-discharge care. Proactively identify and resolve delays, manage discharge planning, and facilitate referrals to ensure efficient resource utilization and timely disposition. Mentor new hires and assist in precepting student interns while maintaining accurate clinical and psychosocial documentation. Participate in interdisciplinary rounds to confirm discharge timelines and recommend best levels of care transition.
