Job Summary

Assess transitional care needs, coordinate care across the continuum, and engage with patients and families to assure care needs are met for complex cases during acute admission. Collaborate with physicians, utilization review nurses, and interdisciplinary teams to drive optimal treatment plans, streamline care progression, and resolve barriers to discharge. Manage all aspects of discharge planning, including psychosocial assessments, insurance coverage requirements, and post-acute referrals, while participating in interdisciplinary rounds to confirm estimated discharge dates. Mentor new hires and student interns in clinical progression and case reviews. Work a full-time 12-hour daytime shift in the Emergency Department at Cobb Hospital.

Required Qualifications

  • Minimum 3 years of experience in healthcare in the acute care setting, related field, skilled care or community environment in care coordination
  • Minimum 2 years in care coordination in the acute care setting
  • Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing
  • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
  • Minimum 3 years experience as a staff nurse in an acute care hospital setting
  • Minimum 2 years experience as a case manager in a hospital setting or payer-based model with expertise in case management competencies and to guide the care team through complex discussions
  • Excellent written and verbal communication skill
  • Knowledge regarding local, state and federal regulations
  • Knowledge of community and state-wide resources and programs
  • Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist through the continuum of care
