Job Summary
Assess complex patient transitional care needs and coordinate care across the continuum to ensure safe discharge planning. Provide comprehensive clinical and psychosocial assessments for high-risk patients, identifying barriers to timely disposition and documenting progress in the Electronic Health Record. Partner with interdisciplinary teams, physicians, and families to facilitate post-acute referrals, manage financial needs, and support end-of-life care decisions. Mentor new care coordinators and student interns on challenging case reviews and social determinants of health. Precept new hires and assist leadership in negotiating agreements with community agencies to optimize resource consumption.
Required Qualifications
- Minimum 3 years of experience in healthcare in the acute care setting, related field, skilled care or community environment in care coordination
- Minimum 2 years in care coordination in the acute care setting
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- Excellent written and verbal communication skill
- Must possess maturity, self-confidence, objectivity, and positive attitude
- Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
- Strong assessment, interview, organizational and problem-solving skills
- Knowledge regarding local, state and federal regulations
- Knowledge of community and state-wide resources and programs
- Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist through the continuum of care
- All certifications are required upon hire unless otherwise stated
