Job Summary
Coordinate patient admissions and scheduling for the hospice referral center, managing communication between referral sources, care providers, and families to ensure timely service delivery. Resolve scheduling conflicts and address customer concerns through direct interaction with patients and the Manager of Access. Provide clinical follow-up on hospital discharges and manage pending patient updates. Requires 2-3 years of hospice experience and strong assessment skills. Full-time 40-hour schedule with rotating shifts and every-third-weekend rotation.
Required Qualifications
- RN required
- 2-3 years of hospice experience required
- Must have well developed nursing assessment, clinical skills and communication skills
- Must be able to lift 50 lbs
