Job Summary
Conduct skilled nursing care for patients requiring professional services, including admission assessments, physical examinations, and implementation of physician-ordered treatments. Prepare OASIS documentation, nursing care plans, and weekly clinical notes while supervising home health aides and evaluating staff performance. Coordinate total client care by consulting with physicians, participating in case conferences, and collaborating with multidisciplinary teams such as therapists and social workers. Travel to prospective client residences to deliver comprehensive care plans and provide education to families regarding home management needs.
Required Qualifications
- Graduate of an approved school of professional nursing
- Licensed Registered Nurse in the applicable state
- Minimum of one (1) year of experience as a Professional Nurse
- Able to communicate verbally and in writing to the extent required by the position
- Able to physically perform the duties required by the position
- Must have a reliable means of transportation and required insurance coverage and a state driver's license
- Must have valid state specific RN license
- Able to travel to prospective clients' residences
