Registered Nurse (RN)- 3P-11P

Conduct physical and psych-social support for residents and families while collaborating with providers and the interdisciplinary team. Perform clinical duties including administering medications, maintaining infection prevention practices, and monitoring resident conditions to ensure safety. Establish care goals and teach residents and families self-care skills, then coordinate a comprehensive holistic plan of care. Ensure documentation is current and accurate, participate in care conferences, and work alongside care associates for routine nursing care. Maintain relationships using a consistent caregiver approach that prioritizes resident independence and preferences.
