Registered Nurse (RN)- 3P-11P

Conduct physical and psych-social support for residents and families, promoting and restoring health through collaboration with providers and the interdisciplinary team. Perform clinical duties per state nurse practice act, including administering medications, maintaining infection prevention, and monitoring resident conditions to ensure safety. Establish care goals and coordinate comprehensive holistic care plans while documenting electronic medical records accurately. Monitor clinical status changes, participate in care conferences, and work alongside care associates to support resident independence and self-care skills. Maintain current, accurate documentation and support resident preferences in their electronic medical record.
