Registered Nurse (RN)- 3P-11P

Collaborate with providers and the interdisciplinary team to promote, restore, and maintain resident health and wellbeing by providing physical and psych-social support. Perform clinical duties per state nurse practice act, including administering medications, maintaining infection prevention practices, and monitoring care conditions to ensure resident safety. Establish care goals and teach residents and families regarding self-care skills while documenting electronic medical records accurately. Participate in care conferences and coordinate holistic plans of care in alignment with the neighborhood team. Maintain relationships with residents and families using a consistent caregiver approach focused on placing the resident first.
