Job Summary
Assess, monitor, and document patient symptoms using required software while collaborating with interdisciplinary teams to develop individualized care plans. Serve as a patient advocate by explaining rights, grievance procedures, and reimbursement processes, and evaluate physical, emotional, and safety status to determine caregiver education needs. Supervise and direct aides and licensed practical nurses in accordance with guidelines, and participate in an on-call rotating schedule to provide back-up assistance. Requires RN licensure with hospice or home health experience preferred.
Required Qualifications
- Registered Nurse licensed in the state of operation
- Nursing experience
- Sound knowledge of nursing principles, procedures and elements of patient family teaching
