Job Summary
Manage case management for assigned patients by performing wound care, IV infusions, re-certifications, and comprehensive admission assessments covering physical, psychosocial, and environmental parameters. Formulate individualized care plans based on assessment data and modify them to reflect progress toward realistic outcomes. Provide skilled nursing interventions, practice professional judgment for task delegation, and coordinate care delivery using Right at Home resources and community networks. Requires current State RN license, one year of recent clinical experience, CPR certification, and availability for weekend and holiday shifts.
Required Qualifications
- Graduate of an accredited school of nursing
- one year of clinical experience within the past five years
- completion of an accredited refresher course within the past two years
- Current State RN license
- Knowledge and ability to provide skilled nursing care as defined in the State Nurse Practice Act
- Current CPR certification
- Available for work on weekends and holidays as required
- Meets current health requirements to provide patient care
- Ability to read, write, speak and understand English as needed for the job
- Possess a valid driver's license
- have use of an insured automobile or access to adequate transportation for the job
