Registered Nurse -Richmond, VA

As an RN with Commonwealth Home Care, you will provide comprehensive home-based nursing care per the client's plan of care, including health and psychosocial evaluations, monitoring, health promotion, discharge planning, and education. You will collaborate with a multidisciplinary team (Physical Therapist, LPN, Occupational Therapist, Speech Therapist, Medical Social Worker) to ensure high-quality, patient-centered care, participate in weekly case conferences and staff meetings, manage documentation and progress notes within required timelines, and support ongoing staff orientation and professional development. The role requires prior home-health experience, strong organizational and teamwork skills, and the ability to perform OASIS-related activities and coordinate care across settings; a valid RN license, CPR certification, and reliable transportation are essential.
