Job Summary
Establish positive working relationships with LTC home staff to develop alliances and demonstrate best practice approaches for older adults with mental health issues, particularly dementia care. Provide specialized geriatric psychiatry nursing interventions, including preparation for clinics and joint practice partnerships with psychiatrists, while assisting staff in creating behavioural care strategies and monitoring resident responses. Act as a consultant to management teams to address systemic problems and support transitions and communication between LTC homes, inpatient services, acute care hospitals, and specialized behavioural support units. Collaborate with LTC management to identify learning needs, design curricula, and facilitate community conferences to share knowledge across all staff levels. Participate in research activities to measure service impact and validate clinical approaches, while contributing to program development and continuous quality improvement initiatives. Ensure all activities comply with safety policies and The Royal's anti-discrimination standards. Valid driver's license required.
Required Qualifications
- Registered Nursing Diploma
- Current unrestricted registration with the College of Nurses of Ontario
- Minimum two (2) years experience in Geriatric mental health nursing
- Valid G driver's license
- Access to a reliable vehicle for work use
- English level A- in oral expression, oral comprehension, reading comprehension and written expression
Desired Qualifications
- Baccalaureate in Nursing
- Two (2) years current experience as a Registered Nurse in LTC
- CNA Certificate in Psychiatry and Mental Health Nursing and/or Geriatric Nursing
- Computer skills and familiarity with Epic electronic health record (EHR)
- Training in BSO Foundations, P.I.E.C.E.S. TM, U-First!, Gentle Persuasive Approach, and DementiAbility
- Willingness to successfully complete training in BSO Foundations, P.I.E.C.E.S. TM, U-First!, Gentle Persuasive Approach, and DementiAbility
- Coaching certifications
- Demonstrated knowledge of the Long Term Care (LTC) Home Act & regulations
- Prior experience with clients in LTC and the community
- Specialized skills in assessment of dementia, delirium and mental health & addictions
- Knowledge of best practices in dementia, delirium and mental health & addictions areas and community resources
- Excellent clinical reasoning, critical thinking, problem solving, physical assessment skills
- Ability to deliver non-pharmacological and pharmacological behavioural management approaches
- Strong verbal and written communication skills
- Ability to work in a collaborative, team model
- Independent practice initiative in organizing and prioritizing work effectively and with minimal supervision
- Flexibility in working hours to meet the needs of clinic schedules with psychiatrists, LTCH staff, residents and their families
- Research and Program evaluation skills
- Demonstrated ability to provide facilitation, coaching, mentoring and adult education skills
- Bilingual (French/English)
