Job Summary
Provide patient assessments and collect health data involving patients, families, and healthcare providers. Coordinate patient care plans to ensure interventions are consistent, safe, timely, and appropriate. Communicate clearly with all stakeholders in person, by phone, and in writing. Individualize patient and family teaching based on thorough assessments to meet specific learning needs. Participate in evaluating nursing practice quality through improvement initiatives and maintain professional knowledge of current standards and regulations. Schedule is 7:00pm to 7:30am with a Level 1 per diem commitment of at least 48 hours every 6 weeks, including 24 weekend hours. No travel required.
Required Qualifications
- Successful completion of an Associate's degree or BSN from an accredited school of nursing
- Current licensure to practice nursing in the State of Illinois
- Current CPR certification from the American Heart Association
- Must be able to work at least 48 hours every 6 weeks
- Must be able to work 24 hours of weekend time
