Job Summary
Conduct patient assessments and collect health data involving patients, families, and healthcare providers. Coordinate patient care plans to ensure interventions are consistent, safe, timely, and appropriate. Communicate clearly with all stakeholders in person, by phone, and in writing. Individualize patient and family teaching based on thorough assessments. Participate in quality improvement initiatives to evaluate nursing practice effectiveness. Maintain professional knowledge of current nursing standards, laws, and regulations. Work per diem shifts from 7:00am to 7:30pm with a commitment of at least 48 hours every 6 weeks, including weekend coverage. Requires an Associate's or BSN degree, Illinois licensure, and current CPR certification.
Required Qualifications
- Successful completion of an Associate's degree or BSN from an accredited school of nursing
- Current licensure to practice nursing in the State of Illinois
- Current CPR certification from the American Heart Association
- Must be able to work at least 48 hours every 6 weeks
- Must be able to work 24 hours of weekend time
