Job Summary
Coordinate multidisciplinary teams to ensure seamless rehabilitation experiences for patients. Create and adapt personalized care plans that drive recovery goals and improve long-term health outcomes. Monitor patient conditions and communicate vital updates to the care team in real-time to support safety and clinical excellence. Guide patients and families through the discharge planning process to ensure a safe transition back to the community. This Registered Nurse Rehabilitation role operates on a Day shift PRN schedule at Ascension Via Christi's Wichita facility.
Required Qualifications
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date
- American Heart Association or American Red Cross accepted certification for BLS
- Licensed Registered Nurse credentialed from the Kansas Board of Nursing
- Diploma from an accredited school/college of nursing
- Professional licensure at time of hire
