Job Summary
Lead implementation and continuous monitoring of personalized care plans to ensure high-standard treatment aligned with patient recovery goals. Coordinate care across multiple clinical disciplines as the central hub for team communication, ensuring alignment toward optimal patient outcomes. Assess and facilitate the patient journey from admission to discharge, collaborating with the healthcare team to enable safe transitions back to the community. Communicate critical changes in patient condition to the medical team in real-time for rapid intervention. This Registered Nurse Rehab role at Ascension Saint Thomas River Park in McMinnville, TN, requires a full-time night shift and BLS certification within 30 days of hire.
Required Qualifications
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing
- current home state license for multi-state license recognition 'Compact State' obtained prior to hire date or job transfer date
- Diploma from an accredited school/college of nursing
- Required professional licensure at time of hire
