Job Summary
Coordinate interdisciplinary care teams to ensure seamless management and optimal patient outcomes. Implement, monitor, and dynamically adjust prescribed care plans based on clinical findings and team input. Precisely interpret and execute all authorized physician orders, including medications, tests, and procedures. Maintain constant vigilance by systematically assessing patients, documenting findings in the EHR, and communicating significant changes to the healthcare team. Proactively assess discharge needs from admission, collaborating with social work and physicians to arrange post-acute resources and provide education for safe transitions.
Required Qualifications
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date
- American Heart Association or American Red Cross accepted BLS certification
- Registered Nurse credentialed from the Wisconsin Board of Nursing
- Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire
- Night shift availability
- 36 hours per week availability
