Job Summary
Provide direct nursing care to hospice patients and manage individualized plans of care while supporting interdisciplinary team members. Assess physical, emotional, spiritual, and environmental needs, accurately document all interventions, and educate families on medications and safety protocols. Collaborate with physicians and referral sources to assist with regulatory documentation and payer compliance. Participate in the on-call rotation for after-hours care and travel to various Gentiva locations to fill urgent staffing gaps. Requires a current RN license, minimum one year of experience, and ability to handle overnight travel within a 100-mile radius or across multiple states.
Required Qualifications
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- Option 1: Overnight travel required to cities within a 100-mile radius
- Option 2: Frequent overnight travel to multiple states is required
- Option 3: Regional travel with overnight stays based on assignment needs
- A company fleet car is provided for eligible employees
