Job Summary
Deliver high-quality, compassionate nursing care for hospice patients while managing individualized plans of care and supporting interdisciplinary team members. Assess and address patients' physical, emotional, spiritual, and environmental needs, accurately document all care provided, and educate families on care instructions and safety. Participate in the on-call rotation to provide after-hours care as needed, collaborate with physicians and referral sources, and assist with regulatory documentation and payer compliance. Travel to various Gentiva hospice locations as assigned to fill urgent census needs, with options including overnight travel within a 100-mile radius, multiple states, or regional assignments based on assignment needs.
Required Qualifications
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- Monday–Friday schedule
- Occasional on-call responsibilities
- Overnight travel to cities within a 100-mile radius OR frequent overnight travel to multiple states OR regional travel with overnight stays based on assignment needs
- Safety-sensitive position eligibility
- Minimum requirements to be eligible for benefits
