Job Summary
Conduct initial and ongoing patient evaluations to determine nursing needs, then initiate and revise plans of care requiring specialized skills. Deliver preventive and rehabilitative nursing procedures while preparing clinical progress notes and care conference summaries per agency policy. Maintain core nursing competencies to ensure effective, efficient, and continuous home-based patient care. Requires current Washington State RN license, one year of recent RN experience, and CPR certification. Recent LTC/SNF/rehab experience is strongly preferred.
Required Qualifications
- current, valid RN license or temporary RN license through the Washington State Board of Nursing
- compact RN license
- valid licensure to practice as a RN
- CPR
- one(1) year recent, verifiable experience as a licensed RN
Desired Qualifications
- Recent licensed experience in LTC/SNF/Rehab
